Part-time Case Manager
Local non-profit agency is seeking a part-time case manager to work with the homeless population. Responsibilities include in-home case management to build independent living skills. Preferred minimum two-year degree in social work, human services, or similar field or related experience.

Position Title: Case Manager
Position Description: Provide case management according to program and grant guidelines to families and individuals.
Minimum Qualifications: Two-year degree in a social or behavioral science or a related field and two years of experience in social work; or an equivalent combination of related education and experience. Strong communication skills. Travel is required.
Responsible to: Executive Director
Position Responsibilities:
Provide case management according to program and grant guidelines to families and individuals.

Provide assistance to callers and conduct intake to determine program eligibility.

Develop case plans with clients which will assist them in attaining goals and becoming self-sufficient.

Encourage personal growth within clients through teaching and modeling the skills needed for clients to become self-sufficient.

Use sound judgement to diffuse hostile situations created by clients.

Track and manage outcomes of clients using appropriate computer reporting systems and case notes.

Develop and maintain knowledge of the different funding sources and requirements of grants to be able to request funding and appropriately administer grants.

Develop and maintain relationships within the community to better serve clients of transitional housing.Ex.Landlords, media, community leaders.
Report to the Executive Director

Meet with Executive Director on a regular basis and take direction from the Executive Director regarding case management
Other Responsibilities:

Participate and attend continuing education and training activities.Travel may be required in these situations.

Follow safe practices in all work activities to avoid injuries and accidents.

Perform other duties and responsibilities as assigned.
